If you've never seen a chiropractor for jaw pain before, it's completely natural to wonder what the appointment will actually involve. Will it hurt? Will there be popping and cracking? What are they going to look at? How long will it take?
This article walks you through exactly what happens at a first visit to Oregon TMJ โ step by step โ so you can come in knowing what to expect and feeling confident about the process.
The short version: Your first visit is primarily an assessment. We listen, examine, explain what we find, and talk through options. Nothing happens without your understanding and consent. Most patients leave their first visit with a clear picture of what's going on and a realistic plan for moving forward.
Step by Step: Your First Visit
We Listen First
Your first visit starts with a conversation. We want to understand your full symptom picture โ not just your jaw, but your neck, head, sleep, stress levels, and what you've already tried. We ask about your history, when symptoms started, what makes them better or worse, and what your goals are. This isn't a rushed intake process โ we take the time to actually understand your situation.
Assessment of the Jaw
We evaluate how your jaw moves โ opening range, side-to-side movement, whether clicking or catching occurs, and at what point in the movement. We assess the muscles around the jaw for tenderness and tension, and evaluate the joint itself. We may measure your jaw opening with a simple ruler โ this gives us a baseline to track your progress over time.
Assessment of the Neck and Posture
This is where we differ from most TMJ providers. We evaluate your cervical spine โ how it moves, where it's restricted, and how it's loaded. We assess your posture, looking at head position, shoulder alignment, and thoracic spine mobility. The jaw and neck work together, and understanding both gives us a complete picture of what's driving your symptoms.
We Explain What We Find
After the assessment, we take time to explain what we found in plain language. No jargon, no rushing through. You'll understand what's happening in your jaw and neck, why it's producing your symptoms, and what the treatment plan looks like. We answer questions โ as many as you have.
First Treatment (If Appropriate)
If it's appropriate based on the assessment, we begin treatment on the first visit. This may include hands-on jaw mobilization, soft tissue work on the jaw and neck muscles, cervical spine treatment, and โ when indicated โ laser therapy. Everything is done within your comfort level. You direct the pace. Nothing is done without explanation first.
Home Exercise Guidance
We typically send you home with a small number of simple exercises or strategies to support what we did in the visit. These might include jaw mobility exercises, posture reminders, or specific stretches for the neck. Simple, practical, and effective when done consistently.
What Treatment Feels Like
Many patients are nervous about chiropractic treatment for the jaw โ particularly those who've heard chiropractic is always about cracking and popping. TMJ chiropractic care is different from general chiropractic in important ways:
- Jaw mobilization is gentle โ small, specific movements to improve joint mechanics. Not forceful manipulation.
- Soft tissue work on the jaw and neck muscles is similar to therapeutic massage โ firm but comfortable, targeting areas of tension
- Cervical spine treatment may include gentle mobilization or adjustment, depending on the findings and your comfort level
- Laser therapy is completely painless โ a gentle warmth at most
Most patients are surprised by how comfortable the process is, and many notice some improvement in jaw tension or headache by the time they leave the first visit.
How Many Visits Will You Need?
This varies significantly by person, severity, and how long the problem has been present. We don't lock patients into predetermined treatment packages. What we can say is that most patients notice meaningful improvement within the first few visits โ and we reassess and adjust the plan as you progress. The goal is always to help you get better as efficiently as possible, not to extend care unnecessarily.
What to Bring to Your First Visit
Any relevant imaging (MRI, X-rays) if you have it. A list of medications and supplements. Notes on your symptoms and what you've already tried. And questions โ bring as many as you have. We want you to leave your first visit with clarity, not more confusion.
